Pretending that Kim Jung-Un isn't the head of state is just silly. There's no "premier" but Kim is "President of the State Affairs" which is effectively head of state.
I also wonder who is really calling foreign policy shots in the DPRK, although you could ask the same question of the U.S. Highlights the issue with simply naming who's in a post that nominally has that role.
Kim is probably heavily involved in that since he has no lobbyists afaik.
