You literally cannot turn the geiger counter off on a PipBoy in the games, and "literally everyone in the vault was so stupid that nobody had their geiger counter on even accidentally" is a bridge too far. Also, if they were that naive, there's no way Lucy would be able to connect the dots there. And how the fuck did they survive for two centuries without being cautious about radioactivity?
It's fine if the characters are stupid and do stupid things. It's not okay if logic itself is just not considered.
idk just because you can't turn the geiger counter off in the game doesn't mean you can't do it. NPCs always talk about doing stuff with your pipboy that you can never do in game like open doors and do scans and shit. Like all the data retrieval missions like the one where you go to the vault with the plant people imply that the pipboy has some sort of file system but you never get to see it yourself or look at the files.
idk I didn't watch the episode. but I don't think the vault dwellers are that sensitive to radiation considering we have multiple instances of them going out into the surface and not dying.
