there's a very clear difference between acknowledging birth sex and insisting that trans men be referred to as 'female'. Even if it isn't like that in the context of the discussion, this discourse is a long-been-tread topic by trans people and it's entirely unproductive to center the conversation around it, it just facilitates reaction and it really is only info that matters to doctors.
Plus, sex is usually the variable demarcated on paperwork, there's a much more insidious reason of insisting on this immutable variable: a way to ensure people are never able to fully pass in the framework of society.
A lot of reactionary rhetoric, esp academic reactionaries, is rooted in 'technically accurates'. It is the way these 'technically accurates' are presented and taken out of context that make it reactionary.
