I live 10 minutes from a hospital. We have a functioning medical system in my country. Midwives here are licensed, trained, government-funded medical professionals and primary care providers. If there's any chance of complications or it's a high risk pregnancy (which is identifiable far in advance), the midwife is going to be qualified to pass us to an OB so the birth can be done as safely as possible in a medical environment.
