I haven't played in months, but it's the same game with a lot of the rough patches smoothed over. They have added several new stories with npcs via mini-expansions (BOS, Pitt, Settler/Raiders, etc). People seem to like it.
Honestly I preferred dead Appalachia; the map was Bethesda's best since Morrowind and the atmosphere was fantastic. The game was also a broken piece of shit and so that all got missed. The add-ons have really decreased the graphical fidelity of the game, and the new NPCs are more of Fallout 4's nonsense.
Also the game is borderline unplayable without a $14 subscription and all the fun fomo building shit is locked behind mtx unless you've been playing since launch and grabbed seasonal items.
