It has been stated many times that everything past not immediately exploding was a stretch goal. I'd call it a partial success considering it made it all the way to stage sep.
Yeah a lot of media reporting on rocket launches and development is pretty bad in general honestly.
Nothing, just sensors to gather as much data during the flight as possible.
That looks like it was hit by a projectile, not a blast wave. I think I saw something on twitter about chunks of concrete from the launchpad flying about.
