I think the idea is that a guy with no butt needs a very tight belt to hold up his pants. A normal belt with holes will either be too loose or too tight unless you happen to have a waist size that lines up with the holes. This belt lets you set a custom length, so it can be tight without being too tight. Also, its just to get clicks from curious people, the actual website doesn't really mention it.

I don't know. It could be the result of a cut feature, and it was just easier to keep both items or they wanted to keep the garlic for the sake of Wario lore.
