I recall FO4 having behaviors that clearly were artifacts left over from morrowind (like in the trailer, the guy's clipboard defaulted to shield position, when you hide around a corner, enemies with guns walk directly towards that corner as if they had a sword, instead of the shortest distance where they'd have a shot)
While I haven't worked with it myself, I get the sense that it's in a similar place to HL2's SDK; bogged down with decades of functionalities and tweaks made for purposes that are no longer relevant, each made atop another layer of forgotten functionalities, with scraps of code going all the way back to quake.
