    I'm genuinely surprised we haven't had something like this happen in the US. We must of but we just don't know...

      This happens all the time in inpatient medicine. We call it the nursing dose when ordered medications or treatments are ignored or modified. Sometimes it's understandable (nurse gave a dying patient in pain some extra morphine under the table) and sometimes it's not (diabetics don't need insulin, silly doctors). There's literally no way to prevent it. I guarantee you this has happened here. I also guarantee you they probably posted on Facebook bragging about it. Whats surprising to me reading this is in Germany they caught it and it was punished. American nurses almost never lose their licenses. Worse than cops if you ask me.
