The USSR collapsed without setting off a nuclear exchange. The UK collapse is happening without any serious concern of some Tory hitting the Big Red Button. The only instance in which nukes were actually employed involved accelerating the collapse of a rival power. They aren't very useful as defensive weapons, and the rules of MAD only become more pronounced as you lose political capital. Relatively few people in power are crazy enough to believe they'd be insulated against a nuclear exchange.
Trump definitely illustrated the real limits of what a President can accomplish. He slapped every button on his desk, but only a few really did anything.
