At least with an actual 12 gauge you get a stock to help with the recoil. This'll just snap your wrist in half.

So do I need one of those wrist braces that bowlers wear to fire this motherfucker?

It obviously fucks up the dude firing it in the video. I can't conceive of this being practical as is.

      However.

There is some sort of rail protrusion on the grip that could be used to mount a stock, and if you had that and a foregrip of some kind, well t hen you're just taking a roundabout way of circumventing shotgun barrel regulations. As long as you can keep it registered as a "pistol" that is.

Based on the video though it seems that loophole was closed and they are very hard to find now.

I remember a clip of a guy shoulder firing a sawed off .12 and it slapped him in the face like an oar. I can only imagine this thing being the same level of stupid but that clip had a guy who kept saying "yo don't do that, hold it at the hip" and he shoulder fired it anyway.

Slugs kick a lot more than shot, and he was shooting hot handloads. Buckshot would be a lot less violent

if it had a vertical foregrip it might not be so outright hostile to the user. Still impractical as fuck.
