They updated the constitution a few years ago and that requires a referendum like this (on top of the community level meetings to decide what changes to the constitution should be made)
This electoral process happens for many things. Like modifying their constitution. For instance, 3 years ago would be the last time something like this happened.

When they call it "unprecedented", they mean "this was one of the most democratic initiatives ever carried out", not that Cuba is undemocratic normally.
