Also, has she met a nurse before? They all smoke like fucking chimneys because the job is inherently stressful.
A good number of hospitals in the US have started banning smoking outright. It's just another drug test on top of all the other ones but checking for nicotine. You leave the hospitals though and a good chunk of the staff are all smoking. Where I work the CNAs smoke and as long as they don't come back reeking of cigarette smoke I don't give a damn because the smell can trigger breathing issues with some people.
Do people know smoking is bad? Yeah. But a good chunk of healthcare staff are also overweight, is that not basically the same thing in this context? We know that being a healthy weight will allow us to live longer healthier lives, but also, you snack through the day, you eat for comfort, you do a bunch of stuff to deal with the stress, you're super tired when you get home.
