This is my thought. She looks a little different... And so? It's been years...maybe even decades considering how witchers age. Plus she clearly underwent the trail of grasses which is famously harsh. Might change your look a bit. And as you say that time is unlikely to be kind. Fighting, hunting, surviving. Oh no she looks a bit different after all that.
