I had one bug early on. I couldn't load saves with other NPCs nearby. Workaround was to load another save, then load the troublesome one. Ultimately, a bug but an inconsequential one. Resolved itself over time.
I've had maybe 3 crashes and 2 frozen loading screens over a full playthrough where I did almost everything.
What platform are you playing on? PS3 seems to be the worst. PC is infinitely better. I haven't used mods, but there's a lot that fix various bugs.
