Acute radiation poisoning falls off pretty rapidly after 5-10 miles, and fallout is mostly a problem for people downwind of the blast center. If the bomb is air-burst instead of ground burst the complications from fallout go down a lot, too. Like if you're within a few miles of ground zero you're probably fucked, but if you survive the blast wave you're probably fine.
