Politicians throughout history love to do the "I'm just saying, the other guy might try it!" approach to signal the threat of escalation without taking credit said escalation. Its the old Khrushchev gambit of threatening to put nukes in Cuba if Americans don't pull their nukes out of Turkey. Nixon and Reagan also both loved to talk big on nuclear escalation in order to cow the opposition, and their efforts did irreconcilable damage to Detente during their respective terms.
You can argue forever over "who started it", but the intent of the message is clear. Lavrov is indicating that his country is ready and willing to embark in a nuclear war if it suspects NATO wants to do the same.
US Media might spin it as "crazy", but the bottom line is that its just the old MAD message Cold Warriors have been sending each other since Truman bombed Nagasaki.

Russian Foreign Minister Sergei Lavrov said on Wednesday that if a third World War were to take place, it would involve nuclear weapons and be destructive, the RIA news agency reported. Lavrov has said that Russia, which launched what it calls a special military operation against Ukraine last week, would face a "real danger" if Kyiv acquired nuclear weapons.
