My understanding is you can sign up for it retroactively if something happens. For example I left my job on a Friday and started my new job on Monday, so I didn't have insurance for the weekend. If something urgent happened, (I think) I could have signed up for COBRA after the fact and had the incident covered.
why does cobra even exist, literally no one can afford it
It exists so Dems can pretend that health insurance isn't completely dependent on employment
