If your phone is reasonably up to date, the default camera app probably has it built in
My phone's nearly a decade old and just barely runs firefox. Not sure what version of Android it's on but it hasn't updated in years either.
Pretty sure F-Droid has a QR-code reader app that will run on Android 5. Maybe 4.
