There's a lot going on in the comic, so it's kind of confusing me but I think that he wrote alive/deceased to say that it is the same woman. While she is alive she says "listen to fauci" and when she is dead she says she'd do it again. If it weren't labeled, it'd be harder to tell it was supposed to represent the same person.
