Just the website you end up using to make your account on, think of it like choosing something like gmail or protonmail. You can still interact with people no matter which you choose, though some may block certain instances (like mose instances block fascist instances) but that usually won't get in your way. Your account also only works on one but you can migrate your follows likes and followers if you need.
It depends but usually it's best to settle down into one, I used to use a really big instance but I eventually moved to cybre.space because I liked the ui and some people I followed were there. When it comes to interacting with people it's almost never a conscious thing, they just look like accounts all the same.
