Given the immense number of people who use it as a pejorative to refer to mentally disabled people, it is. It's the same as the R word.

This article doesn't mention mental disabilities, or online trolling. Why did you link it?

Here's a wikipedia article about NPD that calls it a disability and frequently refers to pwNPD as narcissists or narcissistic

Well, when it's used to refer to people with narcissistic personality disorder, which happens to be most of the time

Ludicrous. Having narcissistic personality disorder is not a mental disability by any reasonable definition of the term. Furthermore, one can have narcissistic personality traits without having the disorder in full. If I'm calling someone a narcissist or narcissistic, I'm not presuming to actually diagnose them with NPD.

isn't narcissism a pattern of behavior and belief? Doesn't really seem like it belongs in the same category as mental disabilities.

50% of people I ask not to use ableist slurs respond that they didn't know there was a connection between the pejorative and the disability. And that would be all well and good, if it weren't for the fact that the other 50% proceed to say that they were talking about disabled people and they think people with NPD deserve hatred. The "I'm speaking greek" excuse, though, doesn't hold any water if you say it in english.
