Not an urban legend, because Viagra like other medication that can impact blood pressure and blood oxygenation it can lead to tissue death of organs unable to get enough oxygen. Erections lasting longer than four hours can then leads to tissue death due to stagnating blood not properly circulating and delivering oxygen to tissue cells.
I think the biggest issue would be low blood pressure. People combine viagra and poppers a lot and end up dying. Most likely chuds will get heart attacks or something.

Its not an injection, its a needle to allow the blood to drain out through the erect penis.
