I had one of these too, it's specifically the RF box that came with the Mega Drive II. Handy for switching between TV and console without unplugging the wire.
Did you flick the switch? Is it on channel 3 or 4? Well idk what's wrong then, try unscrewing and rescrewing the wires on back

i made the mistake of trying to jb weld the screw-in prongs because they kept falling out. lol
hell yeah. I remember when it went from coax/antennae to RCA, I was like "damn the future is now" even though I was only like 11. nothing to jiggle or screw in/tighten. just color coded plugs. that was like the age I was when I started being the one in the family expected to setup the TV/VCR when there was a new one. fuckin boomers.
I never tried to glue the prongs, and tbh the prongs were much better than those RF connectors where the copper wire in the center would bend or break off and then you were just screwed.
I still have my official SNES version because it's still used in equipment today. I can't even count on RCA anymore!
I didn't have one of these with those prongs, pretty sure they were for older systems like the Atari 2600.
update: just scrolled down and saw this reply (https://hexbear.net/comment/5701033) and i own one of those, idk how to use it tho and i just used the composite cable that came with my mega drive (yes i know that composite is le bad and i should use an SCART cable but i dont own a screen that uses it)
