I think you raise a good point, but also that the modern conception of privacy didn't really take hold until the 20th century. Maybe it never happened literally in the same bed but without locks on the doors (without doors at all in most cases) and with less ambient noises to mask the sounds anyone in the same house would definitely be aware that it was happening.
